01/05/18-Entry Open for Title IX T63 (Survey Form)

The second phase of entry for the 2017-2018 Title IX Reports, the ability to enter the results of the student survey, is available and online at schools.khsaa.org. The entry is open and automatically calculates and confirms the response rate (80%) that is necessary.

This (2017-18) is the year for the statewide Form T63 for student interest surveys and all schools must conduct the required biennial survey of your 9th, 10th and 11th-grade students and if appropriate, enrolled 8th-grade students. The T63 form will allow for the final entry of survey results. The form has been revised and will NOT allow for entry if there is less than an 80% response rate. In addition, any entry of 0 as the interest of the students will have to be confirmed since our audit team has found it highly unlikely that no student in the entire survey audience has an interest in a specific sport or sport-activity.

If your school desires to have the KHSAA use its resources to conduct the survey, please contact Rob Catron on our staff for assistance (rcatron@khsaa.org). The KHSAA will be happy to manage and compile your surveys and Rob will be happy to create an online Survey Monkey Survey which you can administer to your students. If your questions relate to an area outside of the conduct of the survey itself, Rob will be able to re-direct your inquiry.

More information regarding completion of the remainder of these reports will be forthcoming, but it is important to give the membership a head start on the entry.
