The Kentucky High School Athletic Association shall sponsor postseason competition in boys lacrosse, provided that there is sufficient interest of the membership and such is approved by the Board of Control. The Commissioner shall divide the state into geographic sections with the approval of the Board of Control.
The alignment shall be reviewed for changes annually as the Commissioner and staff bring proposed changes to the Board of Control following review. During the early years of this championships, participation may change and require regular adjustments which will be done in the "off season" to allow for seeding plan considerations..
In the interim, newly added or deleted programs will be handled on a case by case basis by the Commissioners office.


As of January 2025, the alignment is as follows (with manual notations to be made during the transition to the KHSAA360/ArbiterSports system):

REGION 2
   Central Hardin High School, Desales High School, Dupont Manual High School, North Hardin High School
REGION 3
   Atherton High School, Louisville Collegiate School, Male High School, Saint Xavier High School
REGION 4
   Ballard High School, Christian Academy-Louisville High School, Eastern High School, Kentucky Country Day High School, Trinity (Louisville) High School
REGION 5
   Cooper High School, Covington Catholic High School, Dixie Heights High School, Ryle High School, St. Henry District High School, Walton-Verona High School
REGION 6
   Collins High School, North Oldham High School, Oldham County High School, Shelby County High School, South Oldham High School
REGION 7
